2. Product changes and packaging

Notify BazarPlaza in advance of planned formulation, production-site, process or packaging changes that affect safety, legality or the agreed specification. Obtain written agreement and update the specification before implementing a contractual specification change. Do not delay urgent action required by food-safety law.

Provide specifications and applicable declarations of compliance for food-contact packaging. Materials must meet Regulation (EC) 1935/2004, applicable good manufacturing practice under Regulation (EC) 2023/2006, and material-specific EU and national requirements. Plastic food-contact materials must meet Regulation (EU) 10/2011 as amended, including applicable overall and specific migration limits, supporting evidence and conditions of intended use. Address recycled plastic and bisphenol restrictions where relevant, including applicable transition provisions.

A response is required. Identify the available evidence of PFAS compliance, state if evidence is not yet available, or explain why the requirement does not apply. For food-contact packaging placed on the EU market from 12 August 2026: provide appropriate evidence of compliance with Article 5(5) of Regulation (EU) 2025/40. Evidence may come from the packaging manufacturer and relevant technical documentation or test results, as appropriate. A separate new test is not imposed on every supplier. Submission without evidence does not constitute approval for supply.
Is primary packaging traceable to its supplier and lot?

Packaging registration and producer responsibility: identify responsibilities for each destination market under applicable packaging law, including Regulation (EU) 2025/40 and its relevant application dates. For supplies involving Germany, determine whether LUCID registration and system participation are required for the supplier, BazarPlaza or another party. A missing number is not an exemption. Provide evidence where required.

Are withdrawal and recall procedures documented and tested?
Can traceability records be supplied to competent authorities on demand?
Enter N/A and explain if not tested.

Maintain food traceability as required by Article 18 of Regulation (EC) 178/2002. If supplied food is suspected to be unsafe or non-compliant in a way requiring withdrawal or recall, notify BazarPlaza without delay, identify affected products and lots, and cooperate in the required action. Fulfil duties to competent authorities and consumers where Article 19 applies. Use the agreed incident escalation route and keep supplier emergency details current. The public business number does not replace an agreed emergency arrangement.

As a contractual service commitment, provide available crisis and traceability documents within four hours of a crisis request, and other requested documents within 48 hours unless a shorter applicable deadline applies. These periods do not permit delaying immediate safety notifications, statutory action or records required on demand.

5. Social responsibility and risk assessment

We commit to the amfori BSCI Code of Conduct as a contractual social standard. This commitment is not a claim of certification or a substitute for audit evidence. We and our upstream suppliers, producers and subcontractors must comply with applicable employment and human-rights laws. Relevant ILO standards provide the contractual minimum where national protection is absent or insufficient; mandatory law remains applicable.

Is a social compliance certificate or audit report available?
Have human-rights and environmental risks been assessed?
Have risks associated with products supplied to BazarPlaza been identified?
Examples include GRASP, SMETA, amfori BSCI or SA8000, as applicable. Describe evidence actually held; do not describe an assessment report as certification. Enter N/A if none.

6. Supplier declaration and supporting documents

We confirm that the answers in this declaration are accurate to the best of our knowledge and that the products supplied meet applicable safety, composition, labelling and other legal requirements in the agreed destination markets. We maintain evidence of compliance and promptly disclose changes that affect this declaration or the agreed specification.

Hygiene and HACCP: where our activities fall within Article 5 of Regulation (EC) 852/2004, we maintain procedures based on HACCP principles, proportionate to the nature and size of our business. For primary production and associated operations covered by Annex I, we meet applicable hygiene and recordkeeping requirements. Equivalent and applicable import requirements must be addressed for non-EU operations.

Hygiene and HACCP activity
Provide process flow diagrams and the relevant HACCP control summary where applicable. A CCP is identified only where hazard assessment requires it. Where an IFS certificate is held and sharing is available, grant BazarPlaza access or provide current verification evidence.

We communicate relevant product, safety and traceability requirements to upstream suppliers. If a customer requirement cannot be met, inform BazarPlaza before supply and obtain an agreed resolution; no agreement may waive mandatory food-safety duties.

Current product specifications
Current certificates and audit reports
Food-contact packaging compliance evidence
Risk-based testing plan and relevant results
HACCP or primary-production hygiene evidence
Applicable packaging registration evidence
Identify any document marked N/A or not attached, and explain. Enter None if complete.

Contractual annexes: only annexes identified below, supplied to the supplier and accepted with this declaration form part of this agreement. Do not rely on an unidentified reference to an earlier Conformity document. If none are agreed, write None.

Write None if no annexes have been agreed.

This version remains applicable until replaced or amended by an agreed version, without limiting current legal duties. Changes to circumstances must be notified promptly.
